Short answer: Medicare Part B may cover cataract surgery that removes a cloudy natural lens and implants a conventional intraocular lens. Medicare also covers one pair of eyeglasses with standard frames or one set of contact lenses after each covered cataract surgery that implants an intraocular lens.
Original Medicare coverage
Part B covers qualifying cataract surgery in a hospital outpatient department, ambulatory surgical center, or doctor setting. Coverage includes the conventional intraocular lens and the medically necessary services associated with implantation. Coverage may depend on local Medicare rules.
Medicare does not generally cover routine eyeglasses or contact lenses. The corrective-lens benefit after cataract surgery is a specific exception. Standard frames are covered. Upgraded frames, premium lens features, and options that exceed the Medicare benefit may create additional charges.
Eligibility and limits
The surgery must be medically necessary and furnished by eligible providers. Coverage for premium intraocular lenses or optional refractive features may be limited. A provider should explain which parts are covered and which upgrades are optional before surgery.
Costs
After the Part B deductible, you generally pay 20 percent of the Medicare-approved amount for the surgeon and facility. A hospital outpatient or ambulatory surgical center charge may apply. The approved pair of standard glasses or contacts is also subject to Part B cost sharing. You pay added charges for noncovered upgrades.
Medicare Advantage
Medicare Advantage plans cover Medicare-covered cataract surgery and the post-surgery corrective-lens benefit. Network and prior authorization rules may apply. Some plans also offer separate routine vision benefits, but those benefits are plan-specific.
What to do next
- Confirm that the surgeon, facility, and eyewear supplier accept Medicare or participate in your plan.
- Ask whether the proposed lens is conventional or includes noncovered premium features.
- Request separate estimates for the surgeon, facility, lens, anesthesia, and eyewear.
- Obtain plan authorization when required.
Coverage is determined from the claim, the patient’s eligibility, provider status, and applicable Medicare rules. A provider’s recommendation does not guarantee payment. Before a planned service, ask the provider what will be billed and ask Medicare or the plan whether any coverage review is required.
